Patient Tools and Resources

The 100,000 Lives Campaign

Patient Tools and Resources

To help individuals and organizations learn more about improving the quality of health care and participate in the Remaking American Medicine campaign we have created this Resources section. Here's a brief overview of what you can find here:

  • U.S. Health & Human Services' “Hospital Compare” Website: "Hospital Compare" has quality measures on how often hospitals provide some of the recommended care to get the best results for most patients. You will see some of the recommended care that an adult should get if being treated for a heart attack, heart failure or pneumonia, or having surgery. This information helps you, your health care provider, family, and friends compare the quality of care provided in the hospitals that agree to submit data on the quality of certain services they provide for certain conditions. This quality information not only helps you make good decisions about your health care, but also encourages hospitals to improve the quality of health care they provide.

  • CAHPS Program Hospital Search and Rankings: The CAHPS program develops and supports the use of a comprehensive and evolving family of standardized surveys that ask consumers and patients to report on and evaluate their experiences with health care.

  • Dealing with Chronic Illness: Self-management support is the care and encouragement provided to people with chronic conditions to help them understand their central role in managing their illness, make informed decisions about care, and engage in healthy behaviors. We offer links to national chronic illness resources as a start in your research and increased knowledge, and therefore your increased power and control, over a chronic illness.

  • Patient Resources by State: The Remaking American Medicine campaign is working closely with the nation's Quality Improvement Organizations (QIOs). Every state and US territory or possession is served by a QIO. They have been working for more than two decades with CMS to ensure quality health care for beneficiaries. You can use the information provided to identify if your state's QIO is developing a local coalition.
